Insurance Information
Needless to say we cannot provide you with detailed information concerning your insurance coverage. You are responsible for verifying your coverage and informing your physician of any special required provisions such as a second opinion or precertification. If you provide us with the information we will do our best to satisfy these provisions. But please understand that we cannot guarantee if or to what degree your insurance company will reimburse you. Please remember, you are ultimately responsible for the fee.
We will file your insurance claims for you, therefore it is very important that you provide us with the appropriate information concerning your insurance carriers each time you come in for an appointment. We reserve the right to bill you after 60 days of non-collection from your insurance company, when Spectrum Eye Care is a provider on your insurance plan. Otherwise, payment is due at the time of service when you are the direct recipient of the insurance payment.
We participate with many Medical Insurance Plans as well as various Vision Plans. Below is a list of insurance plans with which Spectrum Eye Care currently participates. This list is subject to change. To access your insurance company’s website, click on the company name below. These sites should give you information on your covered services and benefits. Your insurance card should also have a member services telephone number that can also help you get more information about your coverage. Our billing staff would be happy to answer any of your questions concerning insurance at 419-422-6190 option 7.

Why didn't my insurance pay for my exam?
There are two types of insurance that cover eye care. Medical Insurance and Vision Insurance. Spectrum Eye Care must bill the appropriate insurance as legally directed.
Medical Insurance - (Medicare/Medicaid, Medical Mutual, etc.)
This type of insurance provides benefits for the treatment of medical problems including medical eye problems. To obtain coverage under medical insurance, a symptom or complaint indicative of eye disease, eye injury, or a chronic medical condition is the reason for your visit. Although the examination that you receive may be the same or similar to previous visits, the reason for the exam and the doctor's diagnosis dictate how we must bill our patients.
Vision Insurance - (VSP, EyeMed etc)
This type of insurance covers routine vision care (i.e. regular eye exams for glasses and contacts) when no medical eye problem or related complaint specifically exists.
